nikon d-40 warranty question

I've just received my D-40 from Adorama and I'm a little unsure about what I need to send back to be covered by my warranty. Apparently I have two separate papers (Nikon Inc. One Year Limited Warranty) for each lense with the respective model lense printed on the papers, but how about the body? What do I fill out to be covered by the body?
