I totaly appreciate this may mean a lot to you but you have asked for C&C.
I don't like it because you've layered the grandma photo over tthe subjects head, the pic needs straightened and it is clearly photoshoped and looks stuck on. Pp is overdone for me too.
I'm with MiFleur on this. If you level it up and clone out the power lines the photo will have more impact as a photo.
I can also see the other side - it's not everyone's cup of tea being somewhat sentimental and many will hate it with a passion. However, I assume you aren't planning to submit it for a pullitzer prize or to try to sell it on stockphoto. It's a photo for the family and to try out your editing skills.
One thing I noticed was that her shoulders look a bit like snow capped mountains in the distance; think about adding some mountains at the horizon line and blending them in with the shoulders, add some big white clouds in the the sky and see if you can fade grandma's face in with the clouds a bit to make the effect more subtle. It still won't be Pulitzer material but you might enjoy the challenge!
