Really bright spots using speedlight

Hi guys. This weekend I ran into something I'd never experienced before. I'm shooting with a Nikon D850 and a SB-600 Speedlight. My photos had a bright area in most of the shots, particularly when shooting portrait. I had to shoot in landscape most of the night. Everything ended up working out as most issues were fixable in Lightroom. My shutter wasn't too high. The venue was kinda dark. Any suggestions on fixing this next time? Thanks!

The first one looks like it's just a hot spot created by a highly reflective sign. The second one looks like something is physically blocking the speedlight.

No that's not the case. I thought the same at first. It was doing the same thing when I shot people. Their face/neck area would be super bright like on those photos. But if I turn it and shoot landscape it wouldn't be there.
 
Is the SB-600 set to high-speed shutter mode?
