First impressions - 1) - it seems a little dark - underexposed? 2) - the skin colour has a reddish tinge to it in places. 3) - the background is distracting, particularly that large rectangular, dark object growing out of his head and the rectangular bar coming out of the side of his head. You do have to watch your backgrounds in shots like this, other wise you will often end up with what I call "sprouts" - things that appear to grow from various parts of the body, particularly the head. My 0.02¢ FWIW.
well if your goal was to make is surreal than I think you achieved it. My first impression was def over done eyes.... Ditto to what western said about background
Kayser-Fleischer rings are a sign of Wilson's Disease, which involves abnormal copper handling by the liver resulting in copper accumulation in the body. Better get him checked.
