Flash Problems

So I just bought a Yongnuo YN-560 II for my Canon T3i and I can't seem to get it to fire without hitting the test fire button. I have mounted it on the hotshoe and off with a cable and it won't fire. I have tried on my camera's sync speed and nothing. I am pretty new to off-camera flashes so I am wondering if there is any special settings on the camera other than the sync speed that I could look into?

Just to confirm: The flash will fire neither when mounted in the hot hotshoe, or when used with the remote cable?

The first thing to do is borrow another camera with a known good good hotshoe and test the flash with it, and if possible a different flash with which test the hotshoe on your own camera. You can also check the foot and make sure that there's no damage to the contacts.
